While we appreciate your diligence in completing this form, the printed game report form (returned by the referee) is still the official record of game results. However, submitting this form does meet the requirement that you notify the league Commissioner of the game results by the day following each game.

The game card will be used to verify results submitted here. In order to validate submittals via the Internet, please choose a password the first time you complete the form. This password will be used to validate subsequent game reports, so don't lose it.
