Please complete this form to report the results of a summer league women's game. While we appreciate your diligence in completing this form, the printed game report form (returned by the referee) is still the official record of game results. However, submitting this form does meet the requirement that you notify the Division Commissioner of the game results by the day following each game.

The game card will be used to verify results submitted here. In order to validate submittals via the Internet, please choose a password the first time you complete the form. This password will be used to validate subsequent game reports, so don't lose it.

ATTENTION: Red card for violent conduct (fighting) draws automatic three game suspension
effective immediately (minimum penalty), plus a fine, and an appearance before the league's Discipline
and Protest Committee. Other red card infractions involve greater or lesser penalties.  Do not risk
forfeiting future games by playing an ineligible player -- politely check with referee regarding type of
offense recorded on game report sheet, and review HWSA General Rules to determine penalties. 
If there is any question about length of suspension, please contact the summer league Women's Commissioner.
