ATTENTION: Red card for violent conduct (fighting) draws automatic three game suspension effective immediately (minimum penalty), plus a fine, and an appearance before the league's Discipline and Protest Committee. Other red card infractions involve greater or lesser penalties.

Submitting this form does meet the requirement that you notify the coed commissioner of the game results by the day following each game.
