THIS FORM MUST BE SUBMITTED BY Jan 4th at 5 PM AND $100 NON-REFUNDABLE DEPOSIT MUST BE SUBMITTED VIA PAYPAL OR TURNED IN AT LEAGUE MEETING ON Jan 4th; player fees ($35/player) and state registration forms for all NEW players must be received at the PLAYER registration meeting on Jan 27th. Late Forms and Payments will NOT be considered.

HWSA reserves the right to reject any team name that it deems offensive or conflicts with an existing team or club name within the Houston metro area. Player fees for the Spring 2012 season are $35 per player (including field fees). In addition, each team pays the referee $25 in cash per game at the field.
