HWSA Over 30 COED Winter Registration Form

THIS FORM MUST BE SUBMITTED BY January 4th, and all player registration (including signed forms from all players) is due at the league meeting on January 4th.

Games will be played Thursdays at 7:15 and 8:30pm at Houston SportsPlex. There is a $3 entry fee to the park, but you get one drink token with entry.
It is $35 to register each player, HWSA will cover half of the ref fees this season, with a $80 credit to each team at player registration. Teams will be responsible for paying the refs at the fields, $20 a game.
