 TEAM REGISTRATION FORM

 

THIS FORM AND A $100.00 NON-REFUNDABLE DEPOSIT must be received by the team registration deadline (9 January 2002) to guarantee a slot on the spring schedule. For new teams, $50.00 is held as a bond against future fines. The remainder is applied to player registration fees. For returning teams, the entire deposit is applied to player registration fees.

HWSA reserves the right to reject any team name that it deems offensive or conflicts with an existing team or club name within the Houston metro area.  Also, be advised that if you change your team name in the middle of the playing year (1 September – 31 August) you will be charged an additional $50.00 (a state fee).

SCHEDULING PREFERENCES:
Every effort will be made to give your team as many games at the requested location(s) on the requested days as practical.  Please remember that each team will likely have a number of games on other fields/days.  (Check as many as desired.)

Each team MUST have alternate jerseys or pennies.  All jerseys must be numbered and substantially the same color.  Your team is expected to wear the ‘home’ color when listed first on the schedule.
