THIS FORM AND $100 DEPOSIT MUST BE SUBMITTED BY MAY 7 TEAM REGISTRATION DEADLINE; player registration fees must be received by the 27 May player registration deadline to guarantee a place in the summer schedule.

NOTE: Player registration fees are $35 per player (including field fees). Each team pays the referee $25 per game at the field.

Each team MUST have alternate jerseys or pennies. All jerseys must be numbered and substantially the same color. Your team is expected to wear the "home" color when listed first on the schedule. In event of color conflict, home team must change to alternate jerseys.
